This changes a LOT of code but in the end, behavior is the same.
Unfortunately, many functions had to be changed to accomodate but in majority
of cases, to become simpler.
Functions are also removed specifically those that were there to convert an
IPv4 as a host format to a tor_addr_t. Those are not needed anymore.
The IPv4 address field has been standardized to "ipv4_addr", the ORPort to
"ipv4_orport" (currently IPv6 uses ipv6_orport) and DirPort to "ipv4_dirport".
This is related to Sponsor 55 work that adds IPv6 support for relays and this
work is needed in order to have a common interface between IPv4 and IPv6.

Right now there's a single warn_if_unnamed flag for
router_get_consensus_status_by_nickname() and
node_get_by_nickname(), that is nearly always 1. I've turned it
into an 'unsigned' bitfield, and inverted its sense. I've added the
flags argument to node_get_by_hex_id() too, though it does nothing
there right now.
I've removed the router_get_consensus_status_by_nickname() function,
since it was only used in once place.
This patch changes the warning behavior of GETINFO ns/name/<name>,
since all other name lookups from the controller currently warn.
Later I'm going to add more flags, for ed25519 support.

The coccinelle script produced errors with these test files so
convert the remaining cases of smartlist_add to
smartlist_add_strdup by hand.

This is a big-ish patch, but it's very straightforward. Under this
clang warning, we're not actually allowed to have a global variable
without a previous extern declaration for it. The cases where we
violated this rule fall into three roughly equal groups:
* Stuff that should have been static.
* Stuff that was global but where the extern was local to some
other C file.
* Stuff that was only global when built for the unit tests, that
needed a conditional extern in the headers.
The first two were IMO genuine problems; the last is a wart of how
we build tests.

When parsing torrc ExitPolicies, we now warn if:
* an IPv4 address is used on an accept6 or reject6 line. The line is
ignored, but the rest of the policy items in the list are used.
(accept/reject continue to allow both IPv4 and IPv6 addresses in torrcs.)
* a "private" address alias is used on an accept6 or reject6 line.
The line filters both IPv4 and IPv6 private addresses, disregarding
the 6 in accept6/reject6.
When parsing torrc ExitPolicies, we now issue an info-level message:
* when expanding an accept/reject * line to include both IPv4 and IPv6
wildcard addresses.
In each instance, usage advice is provided to avoid the message.
